This notice is to inform patients the practice of Dr. James Galizia closed effective July 26, 2023.

Dr. Galizia’s patients' medical records will remain at Uvalde Medical and Surgical Associates.

If you are a patient and due to this change would like to move to a different provider and would like a copy of your medical records, please sign an authorization for release of your medical records by picking them up at the UMSA office or requesting our office mail them to the physician of your choice. Your medical records are confidential and require a signed consent for release.

For cardiology care, our office recommends the office of Dr. Shawn Ragbir, located at 1038 Garner Field Road at the UMH Outpatient Specialty Clinic.
